I have NOT attended an Arizona Defensive Driving course within the past 12 months. (12 months must pass between the citation dates of dismissed violations.)

My violation is eligible for diversion — it appears on the Arizona Supreme Court’s list of eligible violations and is not a DUI, criminal speeding, or criminal traffic offense.

I am enrolling to dismiss only one (1) traffic violation with this course.

If I hold a Commercial Driver’s License (CDL), the vehicle I was driving at the time of the violation was NOT a commercial vehicle.

I will complete this course at least 7 days before my court date (or court-approved extension date).

I have NOT already paid the citation fine to the court for the violation I wish to dismiss.
